The LPN will provide medical treatment and personal care to individuals with disabilities while assisting professional staff in delivering quality nursing services. They are responsible for administering medications, monitoring vital signs, and participating in interdisciplinary team meetings to support patient outcomes.

Friday, 8:30am - 5:00pm **This is an office-based position that supports our group home operations and requires routine visits to our residential homes. Responsibilities: provide prescribed medical treatment and personal care and services to persons with disabilities by performing the following duties: Assists RN and professional staff in delivering quality nursing services to persons supported. Administers and oversees specified medication processes and procedures. Removes expired medications from circulation and access. Takes historical information and records persons' supported vital signs. Observes person's supported mannerisms and reports adverse reactions to medication or treatment as needed. Advocates for individuals with IDD Participates as an intricate part of the interdisciplinary team for the people we support All other duties as assigned.
Job Requirements Graduate from an accredited nursing program Valid state Licensed Practical Nurse certification Valid driver’s license and auto insurance Ability to lift between 30 and 50 pounds and provide CPR after certification Ability to remain calm in crisis situations Pre-employment screening: Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities) Drug testing Education verification and other credentialing based on position requirements.
