  • Category: Psychological and Social Services JOB SUMMARY Coordinates and delivers client-centered services designed to promote timely access to medical care and medications for HIV/AIDS, as well as ensuring that PLWHA (People Living With HIV/Aids) have linkages to access available social support services. Works collaboratively with hospital case management, outside agencies, providers and referral agencies to identify those who are in need of medical care in order to establish/re-establish access to medical treatment. Key Job Responsibilities Essential Function Initiates medical case management activities and provides services within established policies, procedures and time frames to provide care appropriate to patient population.
  • Key activities include: assessment of needs and support systems development of an individualized care plan coordination of services to implement the care plan ongoing monitoring/re-evaluation/updating care plans as needed client-specific advocacy review of service utilization Identifies designated population without current medical care; works with outside agencies (i.e., homeless shelters, alcohol and drug abuse treatment facilities, etc.), providers and other organizations to link client to medical care. Engages the client/potential client and forms a relationship of trust and understanding in order to promote and communicate the types of services available. Analyzes the clients’ social situation (i.e., health status, substance abuse history, mental health, educational needs, social support, etc.); works to remove barriers or potential barriers to care. Provides adherence counseling to ensure readiness for and adherence to complex HIV/AIDS treatments. Monitors, evaluates and follows-up with clients to ensure they are adherent with medical care. If necessary, re-engages and works through any identified issues. Works collaboratively with hospital case management, outside agencies, providers and referral agencies in a customer responsive and courteous manner, in compliance with iRESPECT Standards and promoting patient/customer satisfaction to identify those who are in need of medical care in order to establish/re-establish access to medical treatment. Provides community services, such as conducting educational and service programs, providing information material, etc. Coordinates and collaborates with hospital case management in maintaining linkage to medical care. Prepares and maintains required documentation, reports, records and files for operational, administrative and compliance purposes within established timeframes. Utilizes computer to enter and retrieve client information; generate statistics, computations, tables or charts. Provides clear, concise, timely written documentation and updates information and computer data as required. Maintains confidentiality of all information in accordance with hospital, HIPAA, compliance and other regulatory requirements. Maintains competency, professional credentials and education necessary to keep abreast and maintain a current knowledge of case management principles, concepts and techniques, best practices and current trends. Attends continuing education, staff development, and mandatory programs. Participates in staff training and performance improvement activities and utilizes the results of the quality of care activities to initiate changes in practices Required for All Jobs
