We are seeking a Registered Nurse Case Manager to join our team at our program in Warminster, PA.
Earn: $43 / Hour + Enhanced Pay Eligible (Additional $2/hour)
In office plus afternoon travel to one or more of our group homes in Bucks / Montgomery / Philadelphia Counties.
As an RN at Merakey, you'll play a vital role in providing direct nursing care for individuals with developmental and physical disabilities. Your responsibilities will include clinical assessments, medication administration, and collaborating with a multidisciplinary team to ensure comprehensive care.
Administer medications and treatments with precision and compassion.
Perform nursing assessments on residents as needed to identify and address health issues promptly.
Collaborate with various healthcare professionals and specialists to coordinate comprehensive care, keeping the nurse manager informed.
Provide health education to residents and their families, including implementing self-medication programs as directed by the nurse manager.
Conduct in-service training for staff to ensure compliance with protocols and procedures, promoting a culture of excellence in care delivery.
Communicate health issues with the interdisciplinary team, actively participating in staff meetings.
Complete quarterly reports maintaining accurate documentation.
Perform other duties as assigned, contributing to the smooth operation of our program.
Travel to group homes in Bucks and Montgomery Counties.
