The LPN travels to patient residences to provide skilled nursing care and maintain accurate electronic medical records. They work under the supervision of an RN to coordinate care and report changes in patient conditions.

Provide direct quality skilled nursing care, either directly or through RN supervision.
Provide education, support and teaching of care to the patient/client, family or other caregivers.
Maintain quality charting and records as required by agency policy and federal and state regulations.
Coordinate services and visits with patient/clients and/or family caregivers, as well as other members of the home health team.
Be knowledgeable of payer source guidelines.
Provide Hospice continuous care if needed.
LPN in good standing with discipline specific licensing/certification agency and able to meet all federal and state licensure requirements. Current CPR certification or ability to obtain within 90 days of hire. Experience Basic level knowledge of EMR (electronical medical records) and email functions.
