The Registered Nurse will conduct routine and as-needed hospice visits to meet patient healthcare needs while collaborating with physicians and the home health team. Meeting the healthcare needs of patients by completing as needed and routine visits Working closely with the home health team, physicians and community partners in order to deliver optimal care Triaging any emergent needs and collaborating with physicians to provide comfort and quality care to each patient Communicating with medical professionals, the home health team, and others Maintaining accurate, up-to-date records Providing services in accordance with agency policies and regulations that define scope of practice Participating in in-service programs Demonstrating strong communication and customer service skill
