The Care Coordinator provides care coordination services to individuals transitioning from acute care settings to support their recovery goals. They collaborate with community partners and internal teams to ensure continuity of care and successful discharge planning.
Employees working at locations operating 24/7 earn a differential. Shift differentials are also available for evenings, nights, weekends and holidays.

Responsible for providing care coordination services to individuals transitioning from acute care settings. This role supports clients in meeting recovery goals through education, outreach, and connection to Medication Assisted Treatment (MAT) and additional community-based supports. Clients may be referred from emergency departments, primary care, crisis response teams, behavioral health providers, acute care facilities, and community health partners.
