About the role

The Patient Care Coordinator facilitates efficient clinic workflow and coordinates care between providers, patients, and community partners. They are responsible for scheduling, case management, triage, and ensuring preventative health measures are maintained.

Adapt Integrated Health Care is looking for a qualified Patient Care Coordinator to join their team! This position works Monday-Friday.

The Patient Care Coordinator is strongly committed to and promotes a strong belief in Adapt Integrated Health Care’s philosophy, purpose and ideals. The Patient Care Coordinator is a health care professional who works in partnership with primary care providers, behavioral health professionals, community partners, patients and patient family members to ensure the best possible outcome for the patient. The PCC primary role is to facilitate and coordinate patient care and back-office workflow to be as efficient and effective as possible.

Required Qualifications

  • High school diploma or equivalent
  • 21+ Current Driver’s License
  • Current CPR certification
  • CCMA or high-level work experience preferred
  • LPN preferred
  • Primary Responsibilities
  • Create an efficient workflow for all team activities; coordinate, research, plan and drive daily huddles to ensure proper appointments and good patient flow.
  • Thoughtfully and accurately schedule patients using a scheduling algorithm to ensure proper appointment placement and time needs.
  • Facilitate appropriate case management, tracking and follow up for all patient care.
  • Follow up and track care on high risk patients.
  • Utilize recall list protocol to facilitate open access scheduling
  • Coordinate Behavioral Medicine visits for chronic conditions and new patients in conjunction with provider schedule.
  • Attend appropriate meeting to facilitate and report on ongoing position updates.
  • Review schedule and initiate chart reviews to ensure preventative health is up to date.
  • Triage as needed
  • Fill in as needed for Medical Assistants
  • Be available to assist the Medical Providers as needed in the clinic
  • Monitor and follow up of provider referrals, diagnostic testing and ER/Hospital follow ups
  • Call or send patient reminders of post-dated diagnostic testing as needed.
  • Dataload health information from outside resources
  • Promote a Culture of Safety; reporting hazards, errors and potential patient safety issues
