Responsibilities

• Provide direct nursing care to residents in a long-term care setting, under the supervision of an RN or physician, including assessment and ongoing monitoring.
• Administer prescribed medications and treatments per care plans; perform routine procedures (e.g., wound care, injections where permitted) and monitor for adverse effects; document all nursing notes in the resident chart.
• Accurately collect and record vital signs; identify changes in condition and report promptly to the supervising nurse; participate in care plan updates and rounds.
• Assist residents with activities of daily living, mobility, hygiene, nutrition, and hydration; support safety and comfort to promote quality of life.
• Maintain infection control practices, proper use of PPE, and safe medication handling; ensure a clean, organized work environment and proper waste disposal.
• Communicate effectively with residents, families, and the interdisciplinary team; participate in care conferences and coordinate with therapy, social work, and other disciplines as needed.

Requirements

• Active LPN license in New Hampshire or eligibility to practice in NH; current standing as required by state guidelines.
• Current CPR/BLS certification.
• Ability to work 12-hour shifts as scheduled (7:00 a.m. to 7:30 p.m.) on Wednesdays and Thursdays and every other weekend; reliable attendance and punctuality.
• Experience in long-term care or geriatric nursing preferred.
• Strong communication, documentation, and teamwork skills; ability to work effectively within an interdisciplinary team.
• Eligibility for facility credentialing and regulatory requirements; able to complete necessary background checks and screenings as part of the assignment process.
