About the role

The Discharge Planner coordinates safe, timely, and patient-centered discharge plans for individuals in the Comprehensive Psychiatric Emergency Program. They work collaboratively with an interdisciplinary team to connect patients with necessary resources for continued treatment and recovery.

Position Overview United Health Services (UHS) is seeking a Discharge Planner to join our Comprehensive Psychiatric Emergency Program (CPEP), supporting patients during critical transitions of care and helping connect them with the resources needed for continued treatment and recovery. In this collaborative role, you'll work closely with Nurse Care Managers, behavioral health professionals, patients, families, and community partners to coordinate safe, timely, and patient-centered discharge plans for individuals experiencing psychiatric and behavioral health emergencies.

Emergency Department, UHS Binghamton General Hospital Primary Work Shift: Multiple shifts available Compensation Range: $27.80 - $41.69 per hour, depending on experience You will be eligible for benefits if you are hired into a regular position with at least 24 scheduled weekly hours. ----- Schedule Opportunities: Day Shift – 32 hours per week Evening Shift – 32 hours per week Education/Experience Minimum Required: Associate in applied science degree in related field or five (5) years experience in acute/tertiary facility and/or community health nursing.
