James River Home Health is actively seeking a compassionate and dedicated PRN Home Health RN Case Manager to support our patients in the Prince William territory. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience.
$43.27 - $52.88 per hour for orientation, training, required meetings, and in-service education
This is a PRN position with eligible compensation on a per-visit basis. Visit rates vary by visit type and will be reviewed with candidates during the interview process.

Provides skilled nursing care to homebound patients in accordance with the established Plan of Care, developing and implementing individualized treatment plans while monitoring and reporting changes in condition. Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team. Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.
Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia At least one year of home health experience preferred Graduate of an accredited nursing school. Must have a valid driver's license and a reliable car available during work hours. OASIS and HCHB experience PLUS!
