The associate will serve as a bridge between patients and healthcare or social service systems while managing an active caseload of dementia patients. They are responsible for screening, assessing, and onboarding patients while coordinating with clinical teams to ensure high-quality service.
Supports patients with dementia and their caregivers. Experience with ageing population care and caregivers is preferred.

Assist individuals, families, and groups in accessing resources; provide informal counseling, support, and follow‑up.
Coordinate frequently with CMS GUIDE Model team members and IU Health providers on behalf of patients.

Required Qualifications
Bachelor’s degree or equivalent years of relevant experience.
0–3 years of experience
Ability to work in a dynamic and complex business environment.
Demonstrated ability to manage multiple tasks, events, or projects simultaneously.
