The Intake Case Manager manages the admission process for residents entering substance use treatment and coordinates care with internal and external stakeholders. They also provide ongoing case management support to the clinical team and ensure accurate clinical documentation.
Description The Intake Case Manager is primarily responsible for managing the admission process for residents entering Behavioral Health Residential Substance Use Treatment. This includes completing all intake procedures upon the resident’s arrival and coordinating care with relevant stakeholders. When not conducting admissions, the Intake Case Manager provides case management support to the clinical team as needed.
Duties/Responsibilities
Welcomes and assist residents adjusting to residential facility.
Assists clients with adjusting to residential program and preventing unplanned discharge in a compassionate and professional way.
Processes admissions of new residents with necessary documentation such as Consents, Releases of Information, ASAM/ ASAM Continuum, Screening Tools and Treatment Plans.
Initiate coordination of care with identified stakeholders such as natural support, TRBHA, Social Services, Legal entities as part of admissions process.
Coordination of care with health plans and internal interdisciplinary team as appropriate.
Provide culturally appropriate modalities in support of client’s cultural preferences.
Takes a Trauma-Informed Care approach.
Supports and assists case management and clinical teams with resident and program needs when not completing admissions.
Assist in coordination of appropriate level of care for each assigned client.
Actively participate in interdisciplinary treatment team meetings.
Attend clinical oversight under Behavioral Health Professional, Clinical Supervisor or Clinical Manager.
Complete accurate clinical documentation same day as date of service.
Transport clients as needed.
Assist in the collection of client breathalyzer readings and/or urine drug screening.
Secure client information per HIPAA standards.
Attends and/or provides in-service training as needed.
Promotes mission, vision, and values of the Axiom Care program.
Promotes inclusion through Culturally and Linguistically Appropriate Services (CLAS) Standards.
Perform other duties as assigned.
