The Breast Health Nurse Navigator coordinates comprehensive patient care by serving as a liaison between patients, families, and medical providers. They assess patient needs, develop care plans, and facilitate access to diagnostic and treatment services across the continuum of care.
Coordinates plan of care for patients, including, but not limited to, appointments, transportation, education, and follow up treatments to ensure timely delivery of diagnostic and treatment services. Coordinates referrals to medical and radiation oncology, physical therapy, and other services as determined by providers. Provide and coordinate education and information to patient and family to ensure a seamless, continuous, and comprehensive continuum of care.
Education: Graduation from an accredited school of nursing. BSN preferred Licensure: Current Licensure as a Registered Nurse RN in Connecticut Minimum 2-4 years’ previous experience in a healthcare related field Must have certification for CN-BN within 12 months of accepting this position, as required by NAPBC Work Schedule: Full Time, 40-hours/week Monday-Friday Day one benefits!
