Franklin Township Partnership Health Center is seeking a compassionate, organized, and patient-focused Licensed Practical Nurse (LPN) to join our healthcare team on a part-time basis (25 hours per week). This role is ideal for an experienced LPN who enjoys working in a collaborative primary care environment and is committed to providing exceptional patient care.
The LPN will support physicians and clinical staff by providing direct patient care, coordinating follow-up services, managing patient documentation, and assisting with chronic disease management programs. The successful candidate will demonstrate excellent clinical, communication, and organizational skills while maintaining a high standard of patient-centered care.
Schedule - 2 (two) - 10 hour days and every other weekend (9a-1p)
Candidates must hold a current New Jersey Licensed Practical Nurse (LPN) license and have graduated from an accredited Practical Nursing program. Prior experience in a primary care or outpatient setting is preferred, along with proficiency in EMR systems.
