  • RESPONSIBILITIES: Provide Case Management (CPST, Crisis) to persons assigned to caseload. Provide goal-directed support and solution-focused interventions. Work hours are flexible to meet the needs of the person served. The majority of contacts happen out of the office and in community settings. Contact with new referral is made within 48 hours of assignment.

  • FUNCTIONS: Will make all contacts as indicated in the tx plan, and if going to be out of the office, makes arrangements with other staff to ensure contact continues. Completes no-show/cancelation log as required in individual program. Weekly contact sheets are turned in at the end of your work week. Progress notes are completed within 24 hours of service. Makes appropriate referrals to PS/PRI/SE and completes required paperwork. Completes assessments within the required timeframe. Will make regular contacts with the tx team EMR scheduler is utilized and kept updated.

  • QUALIFICATIONS: Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience. Excellent interpersonal and teamwork skills Excellent organizational and time management skills Interest and investment in working with adults who experience Severe and Persistent Mental Illness (SPMI) and their families. Have completed or will complete any required training within six months of employment. Must have a current and valid driver’s license.

The employee will interact with many different persons in varying stages of recovery from their mental illness.

Requirements
