Job Description

Metro Physical & Aquatic Therapy is a growing practice delivering high-quality Physical and Occupational Therapy services directly in patients’ homes and community environments. Our clinicians focus on what matters most — helping patients maintain mobility, independence, and overall quality of life where they live. We’re currently seeking motivated Physical Therapists who want the flexibility, autonomy, and meaningful patient relationships that come with home-based care. With multiple homecare coverage areas available, we work with you to build a caseload in locations that fit your schedule and preferences. All levels of experience are encouraged to apply and will train the right new graduate. Job Type: Per Diem Pay: $70-$90 per visit Location: In and around Hempstead Schedule: Flexible! Responsibilities

Accept and treat patients within your designated geographic area Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans Develop a plan of care and treatment goals in consultation with the physician, patient, and family Plan, prepare and implement customized therapy programs to restore function and promote independence Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques Provide timely and accurate documentation for each visit Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning Deliver care in compliance with agency policies and professional practice standards

Qualifications

Physical Therapist Program Graduate Licensed Physical Therapist in the state of New York or eligible. Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance. Ability to travel to patients’ homes within the assigned geographic territory

Additional Information

The anticipated base hourly range for this position is $70-90. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

Candidates must be a graduate of a physical therapist program and hold a valid license in the state of New York or be eligible for licensure. The ability to travel to patients' homes within the assigned geographic territory is required.
