The Case Manager RN evaluates and manages the changing health and service needs of patients as part of an interdisciplinary team. They collaborate with primary care physicians to create and implement written care plans according to established documentation standards.
As an integral member of the CHAT or ACO team, the Case Manager RN evaluates and manages the changing health and service needs of the patient. The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members. Collaborates with the patient's Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
