About the role

The PrEP Navigator identifies high-risk patients, provides HIV prevention education, and facilitates access to PrEP medication and social services. They also conduct community outreach and maintain patient documentation within the EMR system.

Description Summary

The responsibility of the PrEP Navigator is to oversee the clinic-wide effort to identify patients at high-risk for HIV, educate them about pre-exposure prophylaxis (PrEP) as a method for HIV prevention, HIV Testing, connect them to the clinic provider who prescribes PrEP, support patients in accessing and adhering to medication and refer to other needed social services.

Specific duties and responsibilities

All duties are subject to accommodation in accordance with the American Disability Act (ADA). Developing a mechanism to track and trend all activities described in the work plan Providing PrEP information, behavioral risk-reduction counseling and condoms Verifying client risk factors, eligibility and interest in PrEP Linking patients needing PrEP to care and provide education Serving as an initial point of contact for patients in need of PrEP Scheduling follow-up appointments for HIV/STD testing and lab work Providing benefits counseling, assist with health insurance enrollment and help patients access assistance programs PrEP Providing assistance with pharmacies and ensuring access to medication Entering PrEP patient’s documentation into the EMR System Conducting outreach services for high-risk candidates to increase awareness, includes displaying flyers/ brochures, facilitating presentations and distributing information at events such as PRIDE or community forums Creating advertisements on various social media sites (i.e Scruff, Facebook, Eventbrite) to target potential candidates

Excellent writing, speaking and organizational abilities Must be a self-starter with the ability to work independently and as a team member Must possess computer skills Strongly Preferred: Bilingual and/or Multilingual in English and Spanish Must be able to travel to attend meetings/trainings Education and/or Experience: Minimum Associate’s degree or higher or three years related experience

This position requires manual dexterity sufficient to operate phones, computers and other office equipment. The position requires the physical ability to kneel, bend and perform light lifting. This person must have the ability to write and speak clearly using the English language to convey information and be able to hear at normal speaking levels both in person and over the telephone. Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
