About the role

The Restorative Nurse develops and oversees the facility's restorative nursing program to improve resident functional abilities and independence. This role involves collaborating with therapy teams, supervising restorative aides, and ensuring documentation meets regulatory and MDS standards.

RN Restorative Nurse The RN Restorative Nurse is responsible for developing, implementing, and overseeing the facility’s Restorative Nursing Program to assist residents in maintaining or improving functional abilities. This role collaborates with therapy, nursing, and the interdisciplinary team to support mobility, independence, and quality of life in accordance with residents’ care plans and regulatory requirements. Key Responsibilities Develop, implement, and evaluate individualized restorative nursing programs based on resident assessments and therapy recommendations Collaborate with Physical, Occupational, and Speech Therapy to carry over restorative programs following skilled therapy discharge Assess residents for restorative potential and monitor progress toward functional goals Train, supervise, and mentor restorative aides and nursing staff in proper techniques and documentation Ensure restorative programs are carried out consistently and safely Monitor and document resident participation, progress, and outcomes in accordance with facility policy Participate in care planning, interdisciplinary team meetings, and restorative rounds Identify changes in resident condition and communicate appropriately with nursing leadership and providers Ensure restorative documentation supports MDS accuracy and care plan goals Promote resident independence, dignity, and person‑centered care Assist with survey preparation and demonstrate compliance with CMS restorative care requirements Required Qualifications Current, active Registered Nurse (RN) license in good standing in Ohio Minimum of 2 years of nursing experience, preferably in long‑term care or skilled nursing Working knowledge of restorative nursing principles and CMS guidelines Strong assessment, communication, and organizational skills Ability to educate and supervise staff effectively

Preferred Qualifications

Experience in restorative nursing or rehabilitation settings Familiarity with MDS processes and documentation requirements Experience working collaboratively with therapy services Knowledge of electronic medical record (EMR) systems Previous participation in survey preparation or audits

Requirements

Candidates must hold an active Registered Nurse (RN) license in Ohio with at least two years of nursing experience, preferably in long-term care. Proficiency in restorative nursing principles, CMS guidelines, and strong organizational skills are required for this position.
