Occupational Therapist (Home Health)

Acclaim Home Health is seeking a dedicated Occupational Therapist to join our team in Worcester, MA. This role involves providing high-quality, patient-centered care to individuals in their homes, helping them achieve greater independence and improve their quality of life.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for patients in a home health setting.
• Implement therapeutic interventions to improve patients’ motor skills, cognitive functions, and activities of daily living.
• Collaborate with patients, families, and interdisciplinary team members to coordinate care and ensure optimal outcomes.
• Document patient progress and maintain accurate medical records in compliance with company policies and regulatory standards.
• Educate patients and caregivers on adaptive equipment and techniques to enhance daily living.
• Maintain professional knowledge and skills through continuing education and training opportunities.

Requirements

• Current Occupational Therapist license in the state of Massachusetts.
• Experience in home health or outpatient settings preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a patient caseload efficiently.
• Valid driver’s license and reliable transportation.
