Post-Transplant Coordinator
Monitor for rejection, infection, and complications via labs, imaging, and clinical visits.
Manage immunosuppression therapy in coordination with providers and pharmacy.
Educate patients and caregivers on medication adherence, infection prevention, and lifestyle changes.
Coordinate specialty consults, urgent evaluations, and transitions of care.
Maintain regulatory documentation for adverse events, graft loss, and outcomes.
Participate in protocol development, quality projects, and research initiatives.
Support multidisciplinary communication to promote optimal graft and patient survival.
Candidates must hold a current Ohio RN license and possess an associate degree, with a bachelor's degree preferred. A minimum of three to five years of clinical nursing experience is required, specifically in transplant, nephrology, hepatology, or critical care settings.
