Pre-Transplant Coordinator
Initiate, coordinate, and track pre-transplant evaluations and required testing.
Prepare evaluation charts, review histories, and synthesize findings for committee presentation.
Provide education and obtain informed consent for evaluation and listing.
Collaborate with referring providers, cardiology, and multidisciplinary teams.
Ensure accuracy and compliance of UNOS listing data, closure letters, and notifications.
Manage evaluation communication channels (phones, faxes, imaging transfers).
Support regulatory adherence and process improvement in evaluation workflows.
Candidates must hold a current Ohio RN license and possess an associate degree, with a bachelor's degree preferred. A minimum of three to five years of clinical nursing experience is required, specifically in transplant, nephrology, hepatology, or critical care settings.
