The Hospice Home Health Aide provides personal care, emotional support, and assistance with daily living activities to seniors and clients in their homes or facilities. They are responsible for monitoring client conditions, documenting care, and reporting changes to the nursing staff.

Each Hospice Home Health Aide enhances the ability of Eventide’s Seniors/residents/clients to remain at home, living independently for as long as possible by providing personal care, emotional support, assistance with activities of daily living, and basic therapeutic care to Seniors/clients.
Essential Functions
Performs Seniors’/clients’ personal care in Senior’s/client’s home, Eventide nursing home, or other facility, as assigned and instructed including activities of daily living and therapeutic care according to Senior’s/client’s written plan of care as instructed.
Identifies Senior/client/family teaching needs to the nurse to maintain the Senior/client in their own home.
Assists in monitoring the Senior’s/client’s use of equipment according to the written plan of care.
Provides emotional support to Senior/client and family.
Observes and reports changes in the Senior’s/client’s condition and problems to the Senior’s/client’s nurse or supervisor according to procedures.
Documents care provided, observations made, and communications with the visiting nurse on the Senor’s/client’s clinical record.
Adheres to the Senior’s/client’s Bill of Rights and has knowledge of HHC policies and procedures.
Drives from one Senior/client to another, adapting to changing road and/or weather conditions.
Uses strength, agility and dexterity to gain access to each Senior’s/client’s home, carrying needed equipment into and out of the home.
Evaluates emergency situations, performing CPR when necessary based on training.
Must be able to speak, read, write, and understand the English language including medical terminology related to the job.

REQUIREMENTS: : Ability to drive vehicle from place to place, gain access to each Senior/client home and then safely perform the duties of the job while utilizing standard and specialized equipment. Ability to gain access to the work area and then safely perform the duties of the job. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability of 50 - pounds, demonstrating strength and agility at Eventide’s discretion.
AGE
REQUIREMENT: At least age 18.
EDUCATION: High School Diploma or equivalent preferred.
EXPERIENCE: 6 months experience as an aide or caregiver preferred.
Credentials: CNA Certification in good standing as demonstrated by inclusion on the Nebraska State Registry, required at time of hire. Medication Aide training/competency testing prior to client care required for Home Health Care aide positions. CPR required. Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required. All certifications must be kept up to date at the expense of the employee. Must meet Eventide’s vaccination requirements.

Candidates must hold a valid CNA certification in Nebraska, have a valid driver's license, and be able to lift up to 50 pounds. A high school diploma or equivalent is preferred, along with 6 months of experience as an aide or caregiver.
