We are currently seeking a passionate individual who is eager to make a difference in a dynamic healthcare environment. Manage all aspects of assigned caseload including initial and ongoing assessment of clinical needs, develop and implement individualized treatment plans, develop and implement patient and family engagement strategies, anticipate and prevent early withdrawal from treatment, foster good long-term recovery outcomes, coordinates discharge/follow up care. Initiate, facilitate and manage communication with referral source professionals and other collateral professionals involved in assigned cases, as delegated, including referring physicians, therapists, monitoring programs, attorneys, EAPs, judges and probation officers, etc. Perform effectively as an individual, group and family therapist under clinical supervision. Develop and provide therapeutic and educational activities utilizing individual, family and group modalities. Provide on-call coverage and holiday coverage on a rotating basis with other therapists. Assures patient's rights are protected including HIPPA and CFR 42 confidentiality. Writes and sends high quality and timely reports to collateral professionals as authorized by the patient. Develops and implements comprehensive discharge plans/continuity of care plans in concert with FRC treatment team and collateral professional. Documenting patient care in accordance with ASAM, hospital standards, and other accrediting standards. Work with a high degree of professionalism and independence. Participate productively in individual and group clinical supervision, demonstrating consistent professional growth.

Responsibilities

  • Provides patient assessment, treatment planning, and discharge planning.
  • Conducts groups, family, and individual therapy.
  • Engages patient and families in the treatment process.
  • Provides crisis management when indicated.
  • Provides case management services for all persons on the case load.
  • Coordinates clinical information between the team, patient/family, referral sources regarding progress and discharge and aftercare plans.
  • Documentation and implementation of case management services.
  • Implements discharge plan.
  • Coordinates discharges including any transfers to Vista or IOP services.
  • Coordinates the discharge process for patients in safety sensitive positions or who require it with their licensing/monitoring agencies.
  • Assists in the collection of any required data to ensure accurate records.
  • Maintains awareness of ethical issues in mental health/addictions.
  • Assists with Marchman Acts or Baker Acts if indicated.
  • Contributes to and promotes the operational, financial and productivity goal of FRC.
  • Pursue opportunities for professional development and actively participates in hospitality/employee engagement.

Qualifications

Minimum Education and Experience Requirements

  • Masters or Ph.D. in a mental health related field required. A candidate without a Masters Degree may be considered, the candidate must obtain their Masters Degree within six (6) months of hire. Minimum of one year of clinically supervised experience required, with preference in substance abuse disorders or co-occurring disorders. Supervised clinical internship during graduate school qualifies towards the one year. Experience working in addictions and/or behavioral health co-occurring disorders is recommended. Computer skills required. Recovery professional must have a minimum of three years of continuous sobriety immediately preceding and concurrent with hire date.
  • Licensure/Certification/Registration: Florida license eligible in a mental health field (LCSW, LMFT, LMHC, Clinical Psychology) is required. Addiction certification is recommended.
