About the role

The Registered Nurse provides immediate care to individuals in mental health crises through rapid assessment, patient stabilization, and medication administration. They also coordinate medical care from admission to discharge and facilitate wellness groups for clients.

REGISTERED NURSE Rock Valley Community Programs offers transitional and supportive housing, as well as addiction and mental health treatment services, to adults in the Rock County area. We provide these services to homeless male veterans, clients connected to the criminal justice system, and outpatient care to anyone needing assistance with addiction and/or mental health treatment.

Position

The Registered Nurse (RN) provides immediate, compassionate care to individuals experiencing mental health crises. This role focuses on rapid assessment, patient stabilization, medication administration, and ongoing monitoring in a high-acuity environment. The RN collaborates with an interdisciplinary team to support safe, therapeutic interventions, utilizing de-escalation and trauma-informed approaches. Strong clinical judgment, crisis response skills, and a commitment to patient safety and dignity are essential.

Nurse typically works a

Monday-Friday 8:00am-4:00pm schedule and a rotating on-call to assist with coverage within the department. The Registered Nurse provides oversight to the organization and is available by phone in emergencies.

Key

Responsibilities:

Assess the client; collect and document the client's current health status and medical history. Complete and/or review medical records. Make an accurate and ongoing assessment of the client's status and respond appropriately.

Coordination of all clients’ medical care from admission to discharge. Schedule medical appointments as needed.

Evaluate, perform ongoing assessment, and revise plan of care based on new data and the client's condition.

Proper and accurate documentation. Document accurate and ongoing assessment of client status.

Communicate with the psychiatrist regarding client needs, the nursing assessments, and recommendations daily, and as needed, as evidenced by client feedback and observation.

Communicate symptoms, possible medication side effects, and medication changes to all team members promptly.

Attend all medication checks with the psychiatrist.

Review and maintain Medication Administration Records (MARS).

Provide back-up support to unit staff as needed and be in the on-call rotation system for Harpers Place.

Package medications as needed, always ensuring accuracy.

Facilitate a minimum of 2 groups weekly. Topics to include medication, health, wellness, and other pertinent areas.

Accurately transcribe, initiate, and oversee physician orders according to policy and procedure.

Teach CBRF classes. (Medication Administration)

Qualifications

  • Valid state of Wisconsin Registered Nurse license
  • Graduate of an accredited school of Nursing
  • Previous experience in a psychiatric nursing care setting preferred
  • Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care
  • BLS Certification (Preferred)
  • Behavioral Health Nursing: 1 Year (Preferred)

Requirements

Candidates must hold a valid Wisconsin Registered Nurse license and have graduated from an accredited nursing school. Previous experience in a psychiatric nursing setting is preferred, along with BLS certification.
