Millennium Physician Group is seeking to hire a full-time / Case Manager to join the ACO team.

We want someone who supports our top initiative of ensuring an excellent patient experience. How will you make an impact & Requirements The Case Manager is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness.
The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care.
They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in all program areas.
They demonstrate knowledge of growth and development and possess the ability to assess, interact, communicate, and care for age-specific patient populations.

Required Knowledge and Experience Registered Nurse; active and clear license in the State of Florida.
The license must be maintained once employed.
3+ years of Clinical experience with 1+ years of case management experience preferred.
Required Skills and Abilities To perform this job successfully, an individual should have strong computer skills, especially in Excel, and knowledge of EMR systems; Athena knowledge preferred but not required.

Compensation Range: $70,379.00 to $105,569.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
