The Licensed Practical Nurse is responsible for providing direct patient care, ongoing education and reinforcement of care plans, and providing general support in an ambulatory setting under the direction and supervision of the patient’s physician or licensed medical practitioner or designee in accordance with policy and procedure and competency to promote patient health and wellness. This position requires providing services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served.
Qualifications
Education: High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of a nursing program as approved by the State Board of Nursing.
Experience
Minimum of 1 year of nursing experience preferred..
Licensure
- Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding.

Records and relays patient information, symptoms, and concerns to the patient’s medical provider or registered nurse for further triage assessment and to determine a plan of care Assist patients with therapeutic regimens Reinforces established plans of care and age-appropriate interventions Reinforces patient/family teaching plan based on identified health education needs and the condition and age of the patient through approved educational materials or written instructions Administer tasks, treatments, and medications in accordance with validated competencies

Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.
