BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) to join our Hudson County Visits team.
This position provides one-on-one home health visits for adult and geriatric clients throughout Hudson County, NJ, with an immediate opening in the southern Hudson County (Jersey City/Bayonne) area.

Perform skilled physical therapy visits throughout designated territories. Complete comprehensive physical therapy evaluations and ongoing reassessments. Assess mobility, strength, balance, gait, transfers, endurance, and overall functional status. Develop and implement individualized treatment plans using evidence-based interventions. Provide therapeutic exercise, gait training, balance training, manual therapy techniques, and patient education. Collaborate with nurses, physicians, and interdisciplinary team members to coordinate exceptional patient care. Educate patients and caregivers on exercises, safety strategies, fall prevention, and disease management. Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.
Current New Jersey Physical Therapist (PT) license. Graduate of an accredited Physical Therapy program. Home health experience preferred, but not required. Strong communication, organizational, and clinical assessment skills. Ability to work independently and effectively manage a flexible schedule. Experience with electronic medical records (EMR) preferred. Reliable transportation and ability to travel throughout the assigned territory. Why BAYADA? Base Pay, depending upon qualifications, status, and subject to negotiation: $77.00-$83.00 per point ($120,120-$129,480 annual earning potential) $10,000 Sign on Bonus! for full time PT positions.
