RN - Home Health - Assisted Living Facility Based

Responsibilities

Home Health with Less Travel! Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC). Our facility-based RNs enjoy: providing care to our patients within the beautiful confines of the assisted living facilities where our patients reside 1:1 patient care the ability to follow each patient from start to discharge Enhabit RNs work collaboratively with facility wellness staff on a daily basis to ensure strong lines of communication and overall continuity of care for the residents and their loved ones. Additionally, our facility-based RNs consult as needed with the physician and the office giving details about patient care and act as the point of contact for all disciplines involved with providing care to patients. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of our Registered Nurse RN Home Health.

Qualifications

Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director. Medicare home health or hospice experience is preferred.

Requirements

Must possess a valid state driver license Must maintain automobile liability insurance as required by law Must maintain dependable transportation in good working condition Must be able to safely drive an automobile in all types of weather conditions Must possess CPR certification for the healthcare provider
