About the role

The Care Manager MSW facilitates patient care along a continuum by coordinating resources and addressing psychosocial needs. They work collaboratively with patients, physicians, nurses, and the community under the Care Management Practice Model.

The Care Manger MSW will work under the Care Management Practice Model in collaboration with the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. This process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient.

Required Qualifications

  • Master's Degree Social Work, Psychology, Counseling
  • Upon request driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 6 months relevant Social Work experience in a medical group, health plan, hospital, community, post-acute, or medical office/clinic setting.

Preferred Qualifications

  • Certification in Case Management (CCM, ACM) upon hire.
  • Care management and Managed Care experience.
  • Behavioral Health experience in acute or outpatient setting.
  • Utilization management and/or case management highly desirable.
