The Office for Children with Special Health Care Needs is seeking Social Service Clinician II in Floyd County. The Office for Children with Special Health Care Needs provides comprehensive services for children and youth with special health care needs through newborn hearing screening, early intervention, pediatric specialty clinics, service coordination, case management, therapies, and family support. The Office for Children with Special Health Care Needs collaborates with community stakeholders across the Commonwealth to ensure comprehensive services.

Social Service Clinician II provides social work services to children with special health care needs and their families through assessment of client needs and the provision of social services, serves as a member of a multi-disciplinary treatment team, and performs other duties as required.

Responsibilities

and Duties: Provides medical social services for children with special health care needs and their families through ongoing assessment and evaluation of client and family social service needs. Makes referrals when necessary for access to community services, waiver programs, or resources. Obtains insurance information and verifies coverage. Educates and assists with transition planning for the patient and family. Promotes the elimination of barriers to services by working with patients and families to identify and arrange resources as needed, including home visits, arranging transportation, scheduling interpreters, lodging, food, financial assistance, and facilitation of access to care for all provided services. Assists patients with annual financial updates when required. Serves as an advocate for patients and their families by participating with patients and families in meetings for school or other activities; provides information to families regarding child development, patient diagnosis, emergency planning, and vocational rehabilitation; ensures family participation in decision-making. Serves on community committees and acts as a liaison for outreach activities as an Office for Children with Special Health Care Needs (OCSHCN) representative. Maintains a database of resources available in the assigned district. Maintains records of Care Coordination and Social Services provided by documenting in the patient medical record system utilized by the OCSHCN. Performs other duties as assigned. Preferred

Knowledge, Skills and Abilities

Strong self‑motivation and the ability to perform effectively under pressure. Flexible, analytical, and solution oriented. Strong verbal and written communication skills, time management and organizational skills are necessary. Proven leadership and team‑building abilities. A bachelor's degree supplemented by four years of professional social work experience will substitute for the primary education and experience requirements.

If employed by the Cabinet for Health and Family Services or the Department of Juvenile Justice, an employee in this job classification must furnish to the appointing authority within one month of employment, documentation that he/she possesses a valid driver's license. Must maintain any required licensure(s), certification(s), or other credentials for the length of employment in this job classification. Employing agency is responsible for ensuring employee possesses and maintains required licensure(s), certification(s) or other credentials. Working Conditions Performs job duties in an office/facility setting and visits clients and/or families at their residence. Provides emergency services on 24-hour basis, including accepting referrals and completing investigations at any hour without prior warning. As a result of enforced intervention, the worker runs the risk of physical harm. Probationary Period This job has an initial and promotional probationary period of 6 months, except as provided in KRS 18A.111.
