  • Summary: The Physical Therapist provides comprehensive physical therapy services to patients within the acute care hospital setting. This role evaluates, develops, and implements individualized treatment plans to restore mobility, improve functional independence, and support safe discharge planning. The Physical Therapist collaborates closely with physicians, nursing staff, and interdisciplinary team members to optimize patient outcomes, reduce length of stay, and promote patient safety. Care is delivered in accordance with physician orders, regulatory standards, and Trinity Health policies.

  • Responsibilities: Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on physician referrals and clinical findings. Implement therapeutic interventions to improve strength, mobility, balance, endurance, and functional performance in medically complex patients. Monitor patient response to treatment and modify care plans as appropriate to ensure safe, effective progression of therapy. Collaborate with physicians, nursing staff, case management, and other rehabilitation professionals to support coordinated care and discharge planning. Educate patients and family members on mobility techniques, safety precautions, equipment use, and post discharge exercise programs. Document evaluations, treatment sessions, and progress notes accurately and timely within the electronic medical record in accordance with regulatory and organizational standards.

Requirements
