About the role

The Hospice Field RN will conduct patient evaluations, manage admissions, and develop individualized plans of care for patients with life-limiting illnesses. They will also coordinate with the interdisciplinary team and provide education to patients and families regarding disease progression and symptom management.

Choice Hospice is seeking an experienced Hospice RN Evaluator/Admission Nurse to provide compassionate, patient-centered care to individuals with life-limiting illnesses. In this role, you will complete hospice evaluations and admissions, determine eligibility, develop individualized plans of care, and educate patients and families while ensuring a smooth transition into hospice services.

What you'll do

Complete hospice evaluations, admissions, and comprehensive nursing assessments.

Determine hospice eligibility using admission and recertification criteria.

Develop, implement, and update individualized plans of care.

Coordinate care with physicians and the interdisciplinary hospice team.

Educate patients and families on disease progression, hospice philosophy, medications, symptom management, nutrition, and end-of-life care.

Assess physical, emotional, psychosocial, spiritual, and environmental needs.

Evaluate caregiver readiness and provide ongoing support and education.

Supervise and provide clinical direction to Hospice Aides and LPN/LVNs.

Document assessments, admissions, care plans, and clinical progress accurately and timely.

Communicate changes in patient condition to the physician and interdisciplinary team.

Participate in case conferences and quality improvement initiatives.

Perform on-call responsibilities as assigned.

Qualifications

  • Current Registered Nurse (RN) license.
  • Current CPR certification.
  • Valid driver's license and reliable transportation with current auto insurance.
  • Minimum of 2 years of hospice nursing experience preferred.
  • HomeCare HomeBase experience preferred.
  • Hospice experience required.
  • Excellent assessment, communication, and critical thinking skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.

Candidates must hold a current Registered Nurse (RN) license, CPR certification, and a valid driver's license with reliable transportation. A minimum of 2 years of hospice nursing experience is preferred, and hospice experience is strictly required.
