The LPN will dispense medications, perform patient assessments, and maintain accurate medical records within an opioid treatment clinic. They will also collaborate with an interdisciplinary team to coordinate patient care and participate in treatment team meetings.
LPN- OPIOID CLINIC- Per diem Horizon Health is seeking a LPN for our clinic in Petersburg, VA. The OTP LPN delegates, coordinates, and evaluates patient care through collaborative efforts with the interdisciplinary team members and in accordance with the nursing process and the OTP standards of care and practices.
Dispenses Methadone and other medication, per physician orders and per clinic policy Accurately record all patient information in the patient record Completes brief assessment of patient's mental and physical well-being, monitoring for signs/ symptoms associates with licit and illicit drug use Participates in UDS surveillance, per policy Coordinates medication reviews and staffing with agency physicians Completes all required controlled substance inventories to ensure accurate medication counts Conducts patient education programs according to needs identified, including health and wellness education Participates in treatment team meetings, providing feedback to the interdisciplinary team Schedule Saturday's 5a-9a Horizon’s programs have dedicated marketing support and outstanding resources from the nation’s largest psychiatric contract management company. Candidates must hold a current Virginia LPN license. A minimum of two years of experience in an addiction care setting is preferred, and a bachelor's degree is also preferred.
