About the role

Perform comprehensive health assessments and provide acute and chronic care through both in-home visits and telehealth. Collaborate with interdisciplinary teams to develop individualized treatment plans and maintain accurate electronic medical records.

Position Overview ACE Physicians is expanding into Knoxville Tennessee, and we are looking for compassionate and motivated Nurse Practitioners to help bring high-quality, patient-centered care directly to the communities we serve.

This hybrid role provides care through in-home visits and telehealth, allowing providers to build meaningful relationships with patients while delivering personalized care in the environments where patients feel most comfortable.

Key Responsibilities Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments.

Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.

Develop and implement individualized treatment plans tailored to each patient’s needs.

Educate patients and families on health management, wellness, and disease prevention.

Collaborate with physicians, specialists, and interdisciplinary care teams to ensure coordinated care.

Perform minor clinical procedures such as wound care as appropriate during home visits.

Maintain accurate and timely documentation in electronic medical records (eClinicalWorks or similar platforms).

Advocate for patients by providing respectful, inclusive, and equitable care, including individuals with disabilities.

Qualifications

Active, unrestricted Nurse Practitioner license (required). Experience in geriatrics, internal medicine, or primary care preferred. Previous home health or home-based care experience strongly preferred. Experience caring for patients with disabilities is highly desirable. Comfortable delivering care in both in-home and telehealth settings. Excellent communication skills and a collaborative, team-oriented mindset.
