Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, company policies and procedures, and standards of care. *Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services. *Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs. *Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life. *Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers. *Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
*Bachelor's degree in social work or human services field required. *Must possess any certifications/licensures as required by State of employment to practice in long-term care. *Experience working in a Long Term Care facility
