Licensed practical nurse - Floating

Provides Day to Day support of client wellness within the setting Observes and identifies potential client medical or medication concerns Facilities identification of medical concerns to appropriate medical or psychiatric staff or prescribers Assures identified medical concerns are addressed Communicates and collaborates with appropriate Agency staff, medical care providers, and related community organizations to assure coordination of client care and services.

Completes required paperwork related to communication or services provided Coordinates with appropriate residential staff to facilitate transportation to necessary external medical appointments Promotes health and wellness within the residential facility Supports the development of client wellness education and activities Assists Medical Staff and Prescribers Provides Support with vital signs, and other assistance to providers as needed Collaborates with appropriate residential staff on client discharge plans Participates in staff meetings and medical/clinical supervision as directed Participates in relevant Agency meetings and/or trainings Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.
