The Case Manager will conduct face-to-face assessments to evaluate member medical needs and wellness while developing proactive care plans. They will collaborate with interdisciplinary teams to help members access vital services and achieve improved health outcomes.

Required Qualifications · Must reside in New Jersey. Case Load and Travel will be spread across Burlington, Ocean, Monmouth, and surrounding Counties · Must possess reliable transportation and be willing and able to travel up to 85% of the time. Mileage is reimbursed per our company expense reimbursement policy · Active, unrestricted RN license, New Jersey. · 1 or more years clinical experience (hospital, home health, or ambulatory care). · Comfortable working with diverse populations in various settings. · Strong attention to detail, organizational skills, and ability to work independently. · Computer proficiency and experience handling medical records. · Successful completion of the NJ Choice Certification, including Minimum exam score of 80%, Completion of state training modules and Field mentoring component · Ability to work Monday–Friday, 8:00 AM–5:00 PM EST. No weekends or Holidays.
· Assessment, case management, or discharge planning experience. · Managed care or crisis intervention background. · Strong interviewing and communication skills. · Proficiency with MS Office Suite and EMR systems.
