About the role

The Hospice Nurse provides compassionate end-of-life care, focusing on the physical, emotional, and spiritual comfort of patients and their families. They collaborate with a multidisciplinary team to develop and implement individualized care plans while advocating for patient independence and dignity.

Their main responsibility is to help people live as comfortably and independently as possible and with the least amount of pain, in their final days. One of the largest parts of being a Hospice Nurse involves helping patients, and their families, feel more comfortable about death and providing them with the emotional support they need. Our RNs go beyond that and care for the patient’s mind, body and spirit while supporting their families and caregivers as well.

WHAT YOU CAN EXPECT Three 12 hour night shifts a week- 7pm to 7

30am Position will receive additional hourly shift differential for working nights. Function as a patient advocate. Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families. Collaborate with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience. Assess, collect, and analyze comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care. Develop an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously review and revise the plan of care as patient condition warrants. Prioritize and implement the identified plan, coordinate care delivery, employ interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment. Promote patient independence by establishing patient care goals, teaching patients, friends, and families to understand the condition, medications, and self-care skills.

Qualifications

Requirements

Candidates must hold an Associate's Degree in Nursing and possess a valid Registered Nurse license and CPR certification. At least one year of acute care or two years of long-term care experience is required, with hospice experience preferred.
