The Behavioral Health Qualified Professional provides crisis assessment, discharge planning, and care coordination for individuals with mental health or substance abuse diagnoses. They also perform risk assessments and utilize de-escalation techniques within an interdisciplinary team setting.

Perform Case Management functions of linking and arranging for services and referrals. Provide support to individuals with Mental Health (MH) or Substance Abuse issues in a crisis center setting. Complete crisis assessment, discharge planning and care coordination with stakeholders. Complete risk assessments such as the Columbia Suicide Severity Scale and the Violence Risk Assessment. Utilize de-escalation techniques and solution focused interventions Working closely with other clinical/professional staff to maintain communication and providing feedback. Complete all documentation accurately in within the current working shift Working closely with other clinical/professional staff to maintain communication and providing feedback, standardizing procedures and expediting service implementation. All other duties as assigned. Pre-employment screening: Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities) Drug testing Education verification and other credentialing based on position requirements.
