Nurse Practitioner (NP)
The Nurse Practitioner will provide palliative and hospice care to patients, managing complex emotional and clinical needs during end-of-life transitions. This role involves conducting detailed patient assessments and coordinating care plans while traveling between community-based patient visits.

Position Qualifications: 2 years of Nurse Practitioner experience in an acute care environment.
Home Health, Palliative Care, or Hospice Experience Required.
Masters of Science in Nursing.
Active unencumbered OH NP license.
Current driver's license and auto liability coverage.
Detailed assessment skills as part of this role require recertification for Hospice patients.
Interest in a community-based role that requires travel between patient visits.
Drug Enforcement Agency (DEA) number Required.
National Provider Identifier (NPI) Required.

Candidates must hold a Master of Science in Nursing and an active, unencumbered Ohio Nurse Practitioner license. At least two years of acute care experience is required, along with specific experience in Home Health, Palliative Care, or Hospice settings.
