Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing services under the supervision of a Registered Nurse (RN) and in accordance with the client’s plan of care. The LPN plays an essential role in monitoring client health, administering prescribed medications and treatments, documenting observed of progress and collaborating with caregivers, physicians and family members to ensure quality and continuity of care. The ideal candidate is compassionate, clinically skilled, dependable and ready to make a difference in patients’ day-to-day lives.

  • Key Responsibilities
  • Deliver skilled nursing services in accordance with physician orders and the established plan of care
  • Administer medications, injections, wound care, catheter care, and other nursing procedures safely and accurately
  • Observe, assess, and document changes in client’s condition; promptly report findings to the RN
  • Supervisor and/or physician
  • Provide education and support to clients and family members regarding care routines, medications, and wellness strategies
  • Collaborate with the interdisciplinary care team to coordinate home visits, reassessments and discharge planning
  • Maintain accurate, timely clinical notes and electronic documentation per agency policy and regulatory standards
  • Ensure compliance with all federal, state and agency guidelines, including HIPAA and infection control procedures
  • Participate in case conferences, staff meetings and quality assurance initiatives as required

Qualifications

  • Education: Graduate of an accredited LPN/LVN program
  • Licensure: Current LPN license in the Commonwealth of Virginia (in good standing)
  • Experience: Minimum 1 year of nursing experience required; 3 years preferred o home health or long-term care experience preferred
  • Certifications: Current CPR and First Aid certification
  • Transportation: Reliable transportation with a valid driver’s license and proof of insurance
  • Other Requirements: o Ability to travel locally within assigned service area o Strong documentation and communication skills
