About the role

The Case Manager provides professional support to inpatient social workers and facilitates communication with local agencies. They are responsible for managing treatment referrals and coordinating information to ensure successful patient treatment and discharge.

Responsibilities

  • Friends Hospital Philadelphia, PA Case Manager PRN Friends Hospital is the nation's oldest private psychiatric hospital, located on a beautiful 100-acre campus in Philadelphia, PA.
  • Friends Hospital offers a continuum of care including level of care assessments; specially designed inpatient units for adolescents, adults, and older adults; and a private residential rehabilitation program.
  • Friends Hospital is easily accessed via public transportation and has free, safe, on-site parking.
  • Visit us online at: https://friendshospital.com/ The Case Manager: provides quality case management and qualified professional support and services to inpatient social workers as assigned; works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.

Qualifications
