Job Description

Purpose of Job: Improve the overall health of the communities we serve by enhancing access to quality health, wellness, and supportive services as follow: Essential

Duties and Responsibilities

Performs independent visits while operating under standing order procedures for patients outside of the clinic. This may include but is not limited to providing health assessments, medication reconciliation, wound care, lab work, immunizations, and patient education. Works collaboratively with mobile medicine, outreach, and primary care teams to support population health initiatives. Supports health equity team in identifying health outcome initiatives to reduce health disparities for marginalized and excluded patient populations. Leads efforts to coordinate care and perform case management with unengaged patients to improve their health outcomes through creating care plans to improve chronic conditions and achieve more effective personal, social, education and vocational development. Performs patient, family and caregiver education, including motivational interviewing and patient centered health action plans. Collaborates with other resources within organization and in the community to address patient’s social determinant of health needs. Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.

Requirements

Candidates must hold a valid Washington State RN license and possess at least three years of clinical experience. A Bachelor of Science in Nursing is preferred, along with proficiency in electronic health records and triage.

Qualifications

Education/Experience: Valid Washington State RN License. Bachelor of Science degree in Nursing from four-year college or university, preferred. Minimum of three years’ experience in the clinic setting as a Registered Nurse preferred. Experience in triage and experience in the Community Health setting a plus. Experience with electronic health record strongly preferred. Experience with process improvement a plus.

They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Communicating by talking/hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
