The Care Manager provides intensive, post-crisis case management services to support individuals with developmental disabilities in achieving stability. They collaborate with the CTI team to assess needs, develop service plans, and connect clients with community resources.

Job Description

AHRC NYC is seeking a Care Manger for their Critical Time Intervention Program (CTI). The CTI Care Manager works to develop trusting and meaningful relationships with people enrolled in the program and develop and maintain relationships with community supports and resources. The Care Manager will assess service needs and contribute to the development of relevant plans, make referrals, and help people connect with needed supports. SALARY: $70,000 per year...plus a comprehensive Benefit package. See Benefit information below. ESSENTIAL RESPONSIBILITIES: Works in collaboration with CTI team members to support people’s identified emotional, social, vocational and housing needs during and in the aftermath of crisis/emergency episodes. Provides referral and case coordination services to assist people with achieving stability within their specific environment and capacity. Utilizing community resources to assist people Participating in planning for services. Preparing plans as appropriate or needed to augment progress toward phase goals Ensures delivery of intensive, post-crisis case management services that support ongoing crisis recovery. Providing supportive services, either in-person, in-home or other community locations, by telephone or by combination of methods, as needed and appropriate. Follows a trauma informed care approach when providing support and services Performs all duties necessary for compliance with relevant regulations governing service provision, including record keeping and adherence to CTI principles and model. Conducts other related tasks, as assigned.

Qualifications

REQUIRED QUALIFICATIONS: Bachelor’s degree (BA, BS) in a Human Services discipline from an accredited university program required. Work experience with people with developmental disabilities and psychiatric diagnoses. Prior CTI experience a plus Strong interpersonal, written and verbal communication skills. Excellent skills in using MS Office applications as well as agency-specific database systems. Proven capacity to work productively in highly collaborative settings and to coordinate needs of multiple stakeholders. Ability to make decisions independently and to react to unanticipated events in a prudent and timely manner. Flexibility to operate in a rapidly-changing work environment, with ability to adapt and be sensitive to a broad spectrum of locales and circumstances. Must be willing to travel within five boroughs of New York City. Valid NYS Driver’s License a plus. Bilingual a plus.
