Job Summary and Responsibilities RN Case Manager As our RN Case Manager, you will help patients navigate complex healthcare journeys, ensuring seamless transitions, advocating for their needs, and coordinating resources to optimize their health outcomes and promote continued well-being.

Every day you will assess patient needs, develop comprehensive care plans, and facilitate the delivery of appropriate services across various care settings.

You will be expected to manage and supervise care plans according to policies, procedures, and the State Nurse Practice Act.

To be successful in this role, you must be a skilled communicator, an expert problem-solver, and a compassionate advocate, dedicated to patient-centered care and resource management while adhering to established standards.

Requirements

Candidates must hold a nursing degree from an approved school and possess an active Registered Nurse license in North Dakota or a compact state. Additionally, applicants are required to obtain Basic Life Support (BLS) certification and demonstrate strong communication and care coordination skills.

Conduct comprehensive assessments of patients' medical, social, psycihological, and financial needs to identify barriers to discharge and develop individualized care transition plans. Coordinate and facilitate the delivery of appropriate healthcare services, collaborating with physicians, nurses, social workers, insurance providers, and community resources to ensure continuity and quality of care. Advocate for patients and their families, ensuring their rights are respected, their preferences are considered, and they are empowered to make informed decisions about their healthcare journey. Manage patient caseloads, monitoring progress, evaluating the effectiveness of interventions, and making necessary adjustments to care plans to achieve optimal patient outcomes and efficient resource utilization. Educate patients and families on their health conditions, treatment options, available resources, and discharge instructions, preparing them for successful transitions to home, rehabilitation, or other care settings. Maintain accurate and timely documentation of all case management activities, assessments, interventions, and communications, ensuring compliance with hospital policies, regulatory requirements, and payor guidelines. & THIS JOB OFFERS BOTH AN INCENTIVE AND RELOCATION: We are offering a $5,000 Sign-On Bonus in addition to relocation to new hires who meet the eligibility requirements.
