About the role

The Speech Language Pathologist will provide therapy to adult and geriatric patients while maintaining accurate clinical documentation. They are also responsible for establishing individualized plans of care and traveling to nearby facilities as needed.

Responsibilities

Provide physical therapy to adult and geriatric patients
Prepare documentation, progress notes, and discharge summaries
Establish a plan of care in accordance with the patients' needs and capabilities
Travel to nearby centers as needed

Qualifications

Valid SLP License and Credentials
SNF and/or Acute care experience
