The Certified Nursing Assistant provides direct patient care, including personal hygiene and vital sign monitoring, under the direction of an RN. They also perform essential household tasks and maintain accurate documentation of patient conditions and care provided.
Overview The Certified Nursing Assistant performs nursing procedures and patient care tasks as outlined by a patient’s plan of care. This is a Hospice Role, and will cover Bucks & Philadelphia Counties.
Administer patient care under the direction of RN and as instructed by the CNA care plan. Reports changes in the client’s condition and/or needs to the RN Case Manager Provides personal cares, including but not limited to bathing, shampooing, skin and nail care, oral hygiene, shaving & dressing. Performs Household task to essential to health care, including but not limited to meal preparation and feeding, laundry & light housekeeping. Accurately documents provided cares and submits the documents and pertinent observations following Hospice policy & Procedures Attends and/or completed in-service to ensure compliance with the state requirement of 12 continuous education hours per year. Communicates effectively with all members of the interdisciplinary team Other duties as assigned, monitoring patient vital signs, including body temperature, heart rate, respiration rate and blood pressure.
High School Graduate preferred Must have current, Valid Pennsylvania Nursing Aid license Previous Hospice/Palliative experience
Candidates must hold a current, valid Pennsylvania Nursing Aid license and possess a high school diploma. Previous experience in hospice or palliative care is required.
