About the role

Provide compassionate, hands-on care to residents by assisting with daily living activities and monitoring their health status. Collaborate with the interdisciplinary care team to ensure high-quality service and maintain a safe, infection-free environment.

Provide compassionate, hands-on care while assisting residents with activities of daily living, including bathing, dressing, grooming, toileting, and feeding.

Deliver exceptional customer service by responding promptly to resident needs while preserving dignity, privacy, and resident rights.

Monitor residents' health and report changes in condition promptly to the nursing team.

Assist with safe transfers, ambulation, positioning, and mobility using proper equipment and techniques.

Support residents' nutritional needs, hydration, and overall comfort in accordance with individualized care plans.

Maintain a clean, safe, and infection-free environment by following all safety, infection control, and regulatory guidelines.

Collaborate with nurses, rehabilitation staff, families, and the interdisciplinary care team to provide high-quality resident care.

Accurately document resident care and participate in ongoing training, quality improvement initiatives, and professional development.

Qualifications

• Current New York State Certification

Requirements

Candidates must hold a current New York State Certification. The role requires the ability to perform physical tasks such as resident transfers and mobility assistance.
