Our Nurse Practitioners and Physician Assistants deliver specialized Cardiology and Pulmonology care directly to patients in Skilled Nursing, Long-Term Care, and Post-Acute Care communities, helping improve outcomes for medically complex patients while building meaningful relationships with those they serve.

Assigned Territory This position serves patients throughout the Tinley Park area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in Tinley Park, Blue Island, Oak Lawn, and surrounding communities.

Daily travel within the assigned territory is expected to support patient care and operational needs.

Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.

What You'll Do As a Nurse Practitioner or Physician Assistant with Alteas Health, you'll provide specialty Pulmonology services while working collaboratively with physicians, facility staff, and interdisciplinary care teams to deliver high-quality, patient-centered care.

Responsibilities include: Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions.

Develop individualized treatment plans using evidence-based clinical practice.

Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.

Prescribe medications and treatments within your scope of practice.

Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.

Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.

Maintain accurate, timely, and compliant clinical documentation.

Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.

Build strong relationships with facility leadership and clinical teams.

Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.

What We're Looking For Required Qualifications Current, unrestricted Nurse Practitioner or Physician Assistant license in the applicable state(s) of practice.

Current national certification appropriate to licensure.

Current Basic Life Support (BLS) certification.

Minimum six (6) months of Nurse Practitioner or Physician Assistant clinical experience.

Valid driver's license with reliable transportation.

Ability to travel daily throughout an assigned territory.

Strong clinical judgment, communication, organizational, and critical thinking skills.

Experience utilizing Electronic Medical Records (EMR).

Preferred Qualifications

Two (2) or more years of Nurse Practitioner or Physician Assistant experience. Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics. Requirements

Candidates must hold an unrestricted Nurse Practitioner or Physician Assistant license and current national certification. A minimum of six months of clinical experience and a valid driver's license for daily travel are required.
